This page explains how to request the deletion of your account and associated mobile access data from the RIAJ mobile app and website, operated by Margaret Bowen MD PLLC (Restoration is a Journey).

How to Request Account Deletion

To request the deletion of your mobile app account, please contact the clinic directly using one of the following methods:

Please include your full name and the phone number or email address associated with your account so we can verify your identity before processing the request.

What Gets Deleted

Upon verifying your request, we will permanently delete your mobile app login credentials, digital profile data, device information, and completely remove your access to the mobile application.

What We Are Required to Keep

Because Margaret Bowen MD PLLC is a regulated healthcare provider, we cannot completely erase your medical data. We are legally required to retain certain health and treatment records even after an app account deletion request, in strict accordance with HIPAA, 42 CFR Part 2, and North Carolina and Virginia state laws governing medical record retention. The information we are legally mandated to retain includes:

  • Medical and treatment records
  • Intake forms and clinical documents submitted through the app
  • Appointment and visit history
  • Records related to substance use disorder treatment, which carry additional federal retention and confidentiality protections under 42 CFR Part 2

This information is securely retained for the duration required by law. It is kept strictly confidential and is not used for any purpose beyond what is necessary to meet our legal and clinical obligations. It will not be used to re-create the app account you deleted.

How Long It Takes

We process mobile app account deletion requests within 30 days of receiving them.
